Man City goalkeeper Shay Given did his best to keep West Brom at bay
Two goals in three second-half minutes saw West Brom fight back from a goal down to beat Manchester City and reach the fourth round of the Carling Cup.
Jo gave City an early lead with a powerful finish from the edge of the box but the Baggies came back strongly.
Shay Given made a string of saves but he could do nothing to stop Gianni Zuiverloon's snap-shot and a Simon Cox cut in to make it 2-1 soon after.
City's best chance of an equaliser came and went when James Milner was denied.
But by then West Brom could have been further ahead, with Roman Bednar a constant threat for the home side.
Visiting boss Roberto Mancini, who made 11 changes for this match, will point to Saturday's game against Chelsea as the reason for selecting an inexperienced line-up that included three debutants.
But the Baggies were also fielding what amounted to their second-string, yet ran home comfortable winners.
City were ahead at the break, thanks to Jo's thunderous strike after good work from Swedish teenager John Guidetti on his first start, but even that goal came against the run of play.
Given did brilliantly to keep out a Bednar effort with a one-handed save and his stop to keep out a Giles Barnes header from Somen Tchoyi's cross was even better.
West Brom kept coming forward after the break and they were rewarded when Zuiverloon fired home from 20 yards to finally beat Given.
City defender Ben Mee, another of their players making his debut, felt he had been pushed in the build-up but the visitors could have no complaints about Cox's goal, which saw him fire into the same bottom corner after finding space on the edge of the box.
That stunned big-spending City, who progressed to the semi-finals of this competition last season under previous boss Mark Hughes - and who might have been expected to take it seriously again this time around seeing as they are still without a trophy since 1976.
Mancini, who had replaced Hughes in the hot-seat at Eastlands by the time City lost in the last four to Manchester United, had £100m of talent on the pitch by the end as he threw on James Milner and David Silva to try to salvage the game.
But - apart from Milner's injury-time drive, which was well saved by Boaz Myhill - City created little and West Brom deserved to take their place in the last 16.
West Brom assistant manager Michael Appleton:
"It was a great result. Really positive. We made numerous changes and the lads responded well.
"We were second guessing their side bearing in mind they have Chelsea on Saturday but there was still a lot of experience in the side.
"They brought the cavalry on at the end and we were under the cosh for the last 15 minutes but we did enough to win."
Manchester City boss Roberto Mancini on his team selection:
"I don't regret making the changes. I have big problems at the moment and we couldn't take any risks.
"If I had more players available it would be a different team. But when you play like us and Chelsea, on Saturday at 12.45, the time to recover is very short.
"They would need minimum three days so to play tonight would have been dangerous."
"Saturday is more important, the FA Cup is more important. The Europa League is more important.
"I did not regard this as a necessary sacrifice. We wanted to win. That is why I had four senior players on the bench.
"But I knew there was a chance we could lose because we started with six young players.
"I am happy with these guys. We might have lost the game but they can improve. If they do not play tonight, when will they ever play?"
